Container/Tube: Container with 10% formalin
Specimen Volume: Entire specimen 
Collection Instructions: Label container with patient’s name (first and last), date of birth, medical record number, date of collection, and type of specimen. Patient’s chart label may replace the name, date of birth, and medical record number.
Forms: Tissue Specimen Data Sheet in Request Forms in Special Instructions

Biopsy side/site and clinical diagnosis are required.
