Floor to Order:
- LAB893:  Transfusion Reaction notification to Blood Bank

Submit to the Blood Bank:
-Donor unit with intact infusion set
-Transfusion Record

-Transfusion reaction blood

Container/Tube: Lavender top (EDTA)

Specimen Volume: 10 mL

Collection Instructions: Label tube per Patient Identification and Specimen Labeling Policy and Procedure in Special Instructions.
